  • Navy and Marine Corps: Services Continue Efforts to Rebuild Readiness, but Recovery Will Take Years and Sustained Management Attention

    The Navy and Marine Corps continue to face significant readiness challenges that have developed over more than a decade of conflict, budget uncertainty, and reductions in force structure. These challenges prevent the services from reaping the full benefit of their existing forces and attaining the level of readiness called for by the 2018 National Defense Strategy. Both services have made encouraging progress identifying the causes of their readiness decline and have begun efforts to arrest and reverse it (see figure). However, GAO's work shows that addressing these challenges will require years of sustained management attention and resources. Recent events, such as the ongoing pandemic and the fire aboard the USS Bonhomme Richard affect both current and future readiness and are likely to compound and delay the services' readiness rebuilding efforts. Selected Navy and Marine Corps Readiness Challenges Continued progress implementing GAO's prior recommendations will bolster ongoing Navy and Marine Corps efforts to address these readiness challenges. The 2018 National Defense Strategy emphasizes that restoring and retaining readiness is critical to success in the emerging security environment. The Navy and Marine Corps are working to rebuild the readiness of their forces while also growing and modernizing their aging fleets of ships and aircraft. Readiness recovery will take years as the Navy and Marine Corps address their multiple challenges and continue to meet operational demands. This statement provides information on readiness challenges facing (1) the Navy ship and submarine fleet and (2) Navy and Marine Corps aviation. GAO also discusses its prior recommendations on Navy and Marine Corps readiness and the progress that has been made in addressing them. This statement is based on previous work published from 2016 to November 2020—on Navy and Marine Corps readiness challenges, including ship maintenance, sailor training, and aircraft sustainment. GAO also analyzed data updated as of November 2020, as appropriate, and drew from its ongoing work focused on Navy and Marine Corps readiness. GAO made more than 90 recommendations in prior work cited in this statement. The Department of Defense generally concurred with most of GAO's recommendations. Continued attention to these recommendations can assist the Navy and the Marine Corps as they seek to rebuild the readiness of their forces.   • Department of Justice Announces the Use of Body-Worn Cameras on Federal Task Forces

    Today, the Justice Department announced that it will permit state, local, territorial, and tribal task force officers to use body-worn cameras on federal task forces around the nation.  The department’s policy will permit federally deputized officers to activate a body-worn camera while serving arrest warrants, or during other planned arrest operations, and during the execution of search warrants.  The policy is the result of a pilot program launched by the department last October.

  • Electronic Health Records: DOD Has Made Progress in Implementing a New System, but Challenges Persist

    What GAO Found The Department of Defense (DOD) made progress toward implementing its new electronic health record system, MHS GENESIS. DOD deployed the new system to sites in six of 24 planned deployment phases (i.e., waves), which included about 41,600 users (see figure). DOD also improved system performance and addressed issues experienced at the initial sites. Even with this progress, incidents identified during testing—such as system defects—remain unresolved. DOD has not developed plans to conduct additional testing at future sites to ensure the remaining incidents are fully resolved. As a result, unaddressed incidents could lead to challenges at future sites. Actual and Planned MHS GENESIS Deployments, 2017-2023, as of June 2021 Additionally, implementation of MHS GENESIS faced training and communication challenges. Test results and selected system users indicated that training for MHS GENESIS and the dissemination of system change information were ineffective. For example, the users stated that training was not consistent with the “live” system. Further, users reported that there were too many system changes to keep up with and that they were not adequately informed as changes were implemented. As a result, users were unaware of important changes to their roles or business processes, or to system revisions and improvements. These challenges could hinder users' ability to effectively use the system, impede their knowledge of new workflows, and limit the utility of system improvements. Regarding key program risks, DOD identified and was tracking risks and their associated mitigation plans. Why GAO Did This Study DOD relies on multiple legacy electronic health record systems to create, maintain, and manage patient health information. DOD has determined that these systems, implemented over the past 3 decades, require modernization and replacement. The department has sought to replace these legacy systems with a comprehensive, real-time electronic health record. The conference report accompanying the Department of Defense and Labor, Health and Human Services, and Education Appropriations Act, 2019 and Continuing Appropriations Act, 2019 included a provision for GAO to review DOD's electronic health record deployment. GAO's objectives were to (1) determine what progress DOD has made toward implementing a new electronic health record system, and (2) identify the challenges and key risks to MHS GENESIS implementation and what steps DOD is taking to address them. To do so, GAO analyzed test reports, briefing materials, and incident report tracking documents. GAO also held discussion groups with 356 users at selected sites and interviewed relevant officials.

    The Justice Department resolved Tuesday a lawsuit in which an Indiana Army National Guardsman, Captain Christopher Robbins, alleged that The Dufresne Spencer Group, a limited liability corporation doing business as Ashley Home Store, violated the Uniformed Services Employment and Reemployment Rights Act of 1994 (USERRA). Captain Robbins specifically alleged that The Dufresne Spencer Group violated USERRA when it failed to promptly offer him re-employment after a period of active duty military service.

    A firm protested the Drug Enforcement Administration's (DEA) exclusion of its proposal from the competitive range for translation, transcription, and related support services, contending that DEA (1) unreasonably rejected the initial evaluations of proposals and reconvened a new evaluation panel and (2) did not have a valid basis to reject its proposal, since DEA improperly evaluated its bid. GAO held that (1) there was no evidence in the record that DEA's decisions were not made in good faith and (2) DEA's evaluation was reasonable and consistent with the evaluation criteria set forth in the solicitation. Accordingly, the protest was denied.

  • Defense Logistics: Army Has Not Fully Planned or Budgeted for the Reconstitution of Its Afloat Prepositioned Stocks

    At various stages throughout the current operations in Iraq and Afghanistan, the Army has withdrawn equipment from its stored, or prepositioned, stock sets around the world, as well as from its afloat stocks, thus depleting a large portion of its prepositioned stocks. The Army prepositions equipment at diverse strategic locations in order to field combat-ready forces in days rather than the weeks it would take if equipment had to be moved from the United States to the location of the conflict. The Army Prepositioned Stocks (APS) program supports the National Military Strategy and is an important part of the Department of Defense's (DOD) overall strategic mobility framework. The APS program depends on prepositioned unit sets of equipment and sustainment stocks to enable troops to deploy rapidly and train with prepositioned equipment before beginning combat operations. As we testified in January 2007 and March 2006, however, sustained continuing operations have taken a toll on the condition and readiness of military equipment, and the Army faces a number of ongoing and long-term challenges that will affect both the timing and cost of equipment repair and replacement, particularly to its prepositioned stocks. Over the past several years, GAO and other audit agencies have reported on numerous long-standing problems facing DOD's and the Army's prepositioning programs, including a lack of centralized operational direction; unreliable reporting on the maintenance condition of equipment; equipment excesses at some prepositioned locations; and systemic problems with requirements determination and inventory management. In September 2005, we recommended that DOD develop a coordinated departmentwide plan and joint doctrine for the department's prepositioning programs. In February 2007, we reported that while the Army expected to finalize its implementation plan for prepositioning stocks by December 31, 2006, DOD would not complete its departmentwide strategy before mid-April 2007. We recommended that the Secretary of Defense direct the Secretary of the Army to take steps to synchronize the Army's prepositioning strategy with the DOD-wide strategy, to ensure that future investments made for the Army's prepositioning program would align with the anticipated DOD-wide prepositioning strategy. In addition, the John Warner National Defense Authorization Act for Fiscal Year 2007 required the department to establish a departmentwide prepositioning strategic policy by April 2007.Army officials stated that its worldwide APS equipment sets, including APS-3, would be reconstituted in synchronization with the Army's overall equipping priorities when properly funded and in accordance with the official Army worldwide APS reconstitution strategy known as Army Prepositioned Strategy 2015 (APS Strategy 2015). According to DOD officials, the Army's equipping priorities will be based on evolving conditions and operations such as the availability of equipment and duration of operations in Iraq and Afghanistan, for example. As of December 2007, the Army had not established its overall equipping priorities. Additionally, the Army's APS reconstitution strategy is not correlated with a DOD-wide APS strategy, because, according to DOD officials, a DOD-wide prepositioning strategy does not exist. DOD officials explained that the services are responsible for equipping strategies and that the Joint Staff, consistent with current policy, conducts assessments of the services' prepositioned programs to determine their relationship within the DOD-wide strategic context. DOD officials do not believe additional synchronization of strategies is required. According to DOD, the War Reserve Materiel Policy provides ample policy guidance on war reserve materiel requirements and war reserve materiel positioning while the allocation process is outlined in the Joint Strategic Capability Plan. DOD officials believe publication of the War Reserve Materiel Policy and Joint Strategic Capability Plan satisfies the congressionally mandated requirement contained in the John Warner National Defense Authorization Act for Fiscal Year 2007. Nonetheless, as we recommended in our September 2005 and February 2007 reports, a DOD-wide strategy would set direction and a shared foundation for the services' prepositioning programs. Synchronizing a DOD-wide strategy with the Army's prepositioning strategy would ensure that future investments made for the Army's prepositioning program would align with the anticipated DOD-wide strategy. Without a DOD-wide prepositioning strategy, DOD risks inconsistencies between the Army's and the other services' prepositioning strategies, which may result in duplication of efforts and resources. We continue to believe a DOD-wide strategy is needed in addition to broad strategic guidance.
